Passion Care LLC is committed to upholding the rights of every client and ensuring mutual respect and responsibility in the care relationship.

Your Rights as a Client

As a client of Passion Care LLC, you have the following rights, which we are committed to upholding in every interaction and care visit:

    To be treated with dignity, respect, and compassion regardless of race, color, religion, national origin, sex, age, disability, or sexual orientation.

    To receive care that is appropriate to your needs and delivered by qualified, trained caregivers.

    To be fully informed of your care plan, including the services to be provided, the frequency of visits, and the name of the caregiver assigned to your care.

    To participate in the development and revision of your care plan, and to have your preferences and goals respected.

    To refuse any treatment, service, or procedure, and to be informed of the consequences of such refusal.

    To have your personal health information kept private and confidential in accordance with HIPAA and our Privacy Policy.

    To receive care free from abuse, neglect, exploitation, or harassment by any Passion Care LLC staff member.

    To voice grievances, complaints, or concerns without fear of retaliation, discrimination, or interruption of services.

    To be informed of any changes to your care plan, caregiver assignment, or service schedule in advance.

    To receive a copy of your rights and responsibilities, this Notice of Privacy Practices, and any other documents relevant to your care.

    To have access to your own health records and to request amendments to inaccurate or incomplete information.

    To be informed of the costs of services in advance, including any changes to fees.

    To receive care that supports your independence and promotes your highest possible level of functioning.

    To have your cultural, spiritual, and personal values respected in the delivery of care.
  • 15
    To contact regulatory agencies or advocacy organizations regarding concerns about your care without interference.

Your Responsibilities as a Client

To ensure the best possible care experience, we ask that clients and their authorized representatives fulfill the following responsibilities:

    Provide accurate, complete, and current information about your health status, medical history, medications, and care needs.

    Notify Passion Care LLC promptly of any changes in your health condition, medications, or care needs.
  • 3
    Treat all Passion Care LLC caregivers and staff with dignity, courtesy, and respect.
  • 4
    Maintain a safe and accessible environment for caregivers, including informing us of any known hazards.
  • 5
    Follow the agreed-upon care plan and communicate any concerns or desired changes to your care coordinator.
  • 6
    Provide reasonable access to your home at scheduled care times and notify us in advance of any scheduling changes.
  • 7
    Fulfill your financial obligations as outlined in your Service Agreement in a timely manner.
  • 8
    Participate actively in your care and communicate openly with your care team.
  • 9
    Inform Passion Care LLC if you are receiving services from other home care providers.
  • 10
    Understand and comply with the terms of your Service Agreement and these Terms and Conditions.

How to Exercise Your Rights or File a Complaint

If you believe your rights have been violated or wish to file a grievance, please contact our office. You will not be penalized or experience any change in services for exercising your rights or filing a complaint.
